Studies on the Factors Affecting the Admission Period and Decision of Discharge


Abstract
This survey-type study was designed to explore 1) a difference in expected length of stay in hospital(LOS) between civilian and military patients; 2) a difference in their perception of discharge readiness(PDR) betwe
en the patients and their
physicians
in charge; 3) the reasons of non-discharge readiness from the patients and their physicians in charge; 4) the variables affecting LOS for the patients and their physicians in charge; 5) the variables affecting PDR for the patients and the
physicians in
charge. 1159 military patients and their physicians in charge participated in the study from three general military hospitals. Two separate questionnnaires were used to collect data from the patients and from medical records and their physicians
in
charge during August, 1992. The findings were 1) comparing two groups with same diagnosis, LOS for the patients was 6 to 30 times longer than LOS of the civilian patients while for the physicians in charge, 6 to 26 times longer than LOS of the
'civilian
patients; 2) only 17% of the patients and their physicians in charge were agreed for PDR while 91% of the patients and their physicians in charge were agreed for the perception of non-discharge readiness(non-PDR); 3) the reasosns for non-PDR were
the
needs of on-going treatment, rest, physical therapy and observation in order both for the patients and their physician in charge; 4) the variables affecting LOS for the patients were the satisfaction of their military life prior to their
hospitalization
and the length of their military service; 5) the variables affecting LOS for the physicians in charge were the patient's length of military service, the experience of surgical operation, the satisfaction of their military life prior to their
hospitalization and the expectation of follow-up care after hospital discharge; 6) the variables affecting PDR for the patients were health condition, the satisfaction of their military life prior to their hospitalization and their length of
military
service; 7) the variables affecting PDR for the physicians in charge were patient's health condition, the expectation of follow-up after hospital discharge and patient's length of military service.
